Disease of the cerebellum or vestibular system usually results in asymmetry of gaze-evoked nystagmus between directions of gaze. For example, tumors of the cerebellopontine angle may result in high-amplitude, low-frequency nystagmus (caused by cerebellar damage) when looking to the side of the lesion, and low-amplitude, high-frequency nystagmus (caused by vestibular imbalance) when looking to the contralateral side, a condition known as Brun nystagmus. Associated neurologic abnormalities such as ataxia, hearing loss, tremor, or hemiparesis should always be sought. Central vestibular nystagmus is frequently uniplanar in contrast to peripheral vestibular nystagmus, which is usually torsional or multiplanar. For instance, a baby whose parents are first cousins and who has searching eye movements and hypermetropia is liable to have a congenital retinal dystrophy (see Chapter 46). The pediatric ophthalmologist should inquire about other family members, examine them and the first-degree relatives. Examination It is often difficult to quantify visual acuity in babies, especially in those with poor visual contact. Parental estimations of vision and changes in vision can be helpful, but it is relevant to obtain a measure of overall visual function. Despite advances in research, behavioral observations remain of primary importance in assessing visual function in infancy. It becomes thicker as the muscle approaches its insertion and it is usually tightly adherent to the orbital aspect of the sheath of the inferior rectus muscle. Small extensions of the sheath near the inferior oblique muscle insertion are directed to the sheath of the lateral rectus muscle and to the sheath surrounding the optic nerve posteriorly. The inferior oblique muscle and superior oblique tendon enter the episcleral space anteriorly and course posteriorly to insert on the sclera. The rectus muscle pulleys, muscle capsule, and intermuscular capsule are not represented in this diagram. When intracranial complications are present, these take precedence and should be managed by the neurosurgical team. The initial management should be tailored toward treating infection, reducing cerebral edema and draining collections of pus. Close co-operation between otolaryngologist and neurosurgeon is essential; combined or sequential surgery may be necessary when managing conditions such as venous sinus thrombosis and extra dural abscess. If cholesteatoma is present this will need surgical removal either at the time of the initial intervention or shortly afterward. When an extradural abscess is present this may be drained through the dural defect after cortical mastoidectomy and removal of granulation tissue. When the extradural abscess is less accessible or if there is a brain abscess, a burr hole or craniotomy will be required. If a subdural abscess has formed urgent drainage is required, and if there is marked brain swelling part of the cranial vault may need to be removed until this has reduced. Sigmoid sinus thrombosis may be managed using a combination of antibiotics and surgery. During a cortical mastoidectomy the surgeon may find granulations overlying the sigmoid sinus. Sigmoid sinus thrombosis must be suspected; needle aspiration of the sigmoid sinus will confirm the diagnosis (no blood will be aspirated). When a sinus thrombosis is confirmed, either by aspiration or imaging, some surgeons advocate removal of the bone over the sigmoid sinus with opening of the sinus and removal of infected clot and pus.
